Concierge Medical Services for only $150.00 a month

The concierge medical service includes:

•   24/7 Telephone Consultation Unlmted*
•   Up to 5 office visits per month( fee must be paid by the 5^th of the month to be accepted)
•   1 refill of Medical Prescription(by law patient must be seen by the doctor to receive prescription)

When you are accepted to the Concierge Services Program you will receive multiple services at a discounted rate**

Discounts on the following:

•   Blood Test
•   EKG
•   Ultrasound
•   Neurology Consultation
•   Orthopedic Consultation
•   Podiatry Consultation
•   Wound Care
•   Flu Shot
•   Physical Therapy (3 modalities) $40.00 but regularly $85.00
•   Half hour massage for $30 which is normally $90
•   1 hour massage for $40 which is normally $110
•   Work Related injuries on Lien Basis
•   Weight Management Supplements for $130, normally $180
•   Body Wrap Treatment $30 for once a week, normally $60
•   Pain Management Injection $80, normally $250

*   No hospital services will be covered by the office
